Why Screening Matters
Most serious diseases begin silently. Heart disease, type 2 diabetes, bowel cancer, and cervical cancer can all be present for years before causing symptoms — and the window for effective treatment is widest before symptoms appear.
Organised screening programs exist because the evidence consistently shows that finding disease early saves lives:
- Bowel cancer caught at an early stage has a five-year survival rate above 90%; advanced disease falls below 15%.
- Cervical cancer is largely preventable through HPV vaccination and screening — countries with high participation have driven incidence toward near-elimination.
- Breast cancer detected by routine mammography is more likely to be localised, with better treatment options and survival outcomes.
- Blood pressure and cholesterol, when identified and treated early, substantially reduce the risk of first heart attack and stroke.
Balancing Benefits and Harms
Screening is not without tradeoffs. Understanding both sides helps inform decisions:
- False positives — a test suggests disease when none exists, leading to anxiety and follow-up procedures
- Overdiagnosis — detecting slow-growing conditions that might never cause harm, resulting in treatment with no benefit
- False negatives — a test misses existing disease, creating false reassurance
No screening test is perfect. Most programs are designed so that benefits outweigh harms across the population — but individual circumstances vary. Shared decision-making with your clinician is the right approach, particularly for tests like PSA where the evidence is more nuanced.

Cancer Screening
Cervical Cancer
Cervical cancer is almost entirely preventable. HPV vaccination and regular screening together have driven incidence toward near-elimination in countries with high participation.

Who should screen: People with a cervix, typically from age 25 (varies by country). Intervals vary but most HPV-based programs recommend 5-yearly testing in people with a normal result.
Breast Cancer
Mammography is the only screening modality with demonstrated reduction in breast cancer mortality in randomised trials.

Bowel Cancer
Bowel cancer is one of the most common cancers in high-income countries and one of the most preventable through screening.

Who should screen: Most guidelines recommend starting at age 45–50 for average-risk adults, with the FIT test (home stool test) every 1–2 years. Earlier screening applies if you have a family history, Lynch syndrome, or long-standing inflammatory bowel disease.
Prostate Cancer (PSA Testing)
Prostate-specific antigen (PSA) testing is more complex than most cancer screens. It can detect cancer early, but also leads to overdiagnosis and overtreatment of cancers that would never cause harm.

Who should screen: There is no universal programme for PSA screening. Most guidelines recommend an informed discussion between men aged 50–70 (or 45+ with elevated family risk or African heritage) and their clinician, weighing benefits and potential harms. PSA screening is not appropriate as a routine check without that conversation.
Skin Cancer
Skin cancer is among the most common cancers in Australia and other high UV-exposure countries. Most cases are preventable and treatable when caught early.

Who should screen: Australia has no national organised skin cancer screening program, but annual skin checks by a GP or dermatologist are recommended for people with a history of skin cancer, many moles, or significant past sun exposure.
Lung Cancer (Low-Dose CT Screening)
Lung cancer is the leading cause of cancer death globally and in Australia. Low-dose CT (LDCT) is the only screening method with demonstrated mortality reduction — but it is targeted at people with a significant smoking history, not the general population.

Cardiovascular and metabolic conditions are the leading causes of death globally — and many are detectable well before symptoms appear.

High blood pressure is the most common modifiable cardiovascular risk factor. It causes no symptoms until damage is already occurring.

Cholesterol and Cardiovascular Risk

Who should check: Cholesterol screening typically starts at age 35 in men and 45 in women (earlier if family history or other risk factors are present). Lp(a) testing is recommended at least once in life; a high result shifts risk management decisions.
Abdominal Aortic Aneurysm (AAA)
An abdominal aortic aneurysm is a dangerous, symptomless dilation of the main abdominal artery. Rupture is fatal in approximately 80–90% of cases. A single abdominal ultrasound can detect it before that happens.

Who should screen: Men aged 65–75 who have ever smoked (USPSTF 2019 Grade B recommendation). Men aged 65 and over with a family history of AAA, or with established cardiovascular disease, should discuss screening with their GP. Australia has no national AAA screening program as of 2026.
Diabetes and Prediabetes
Type 2 diabetes develops slowly — prediabetes often precedes it by years and is entirely reversible with lifestyle change.

Who should screen: Adults over 40 with one or more risk factors (overweight, family history, gestational diabetes history, hypertension, or certain ethnicities with higher baseline risk). Screening uses fasting blood glucose or HbA1c.
Age-Based Health Checks

In Your 20s
